We lost our Favorite Cat (El Gee) when he adventured out of our back yard and got hit by a car.
This new kitten (who will be a year old in June) has sported a harness since he was big enough to keep it on.
Believe it or not, he is happy to be chained up in the back yard. If he does manage to get wrapped up around a bush or something, he can scrunch up his shoulders and back out of his harness.

Odin wears a harness pretty frequently although he doesn't get to go outside very much.
When the weather gets nicer and my back isn't screaming in pain, I'll take him to the park.

I used to have indoor cats. Then I got Darwin and Roxie.
They were only a few months old when we got them, but they had been raised as indoor/outdoor cats. Darwin figured out the dog door sometime around 30 seconds after he was introduced to our home. Roxie took about 45 seconds.
Our other two cats, Switch and Bondi (who's pure white with blue eyes and is totally deaf) figured it out by watching Darwin and Roxie.
Other than locking the dog door, there's no way to keep them in. Anything big enough to allow a 120 pound puppy through will certainly allow for a cat.
There have been no real issues so far, they stay in my back yard and the neighbors back yards (the hunting is very, VERY good according to the various carnage I find on the floor of my kitchen... the preferred place to eviscerate small woodland creatures). I'm just worried that Bondi will wander out in the road and become a fur pancake.
